Core Support
Assist with everyday activities and the basic support you need to live your life and achieve your goals. This can include house cleaning or having a support worker accompany you on your shopping trips. It may also include daily activities such as assistance with self-care activities, assistance with social and community participation. Assistance with everyday needs, household cleaning and yard maintenance. Everyday items you may need. For example, continence products or low-cost assistive technology and equipment to improve your independence.
Our role in Plan Managed
Engage the participant in decision making regarding their plan. We ensure to use the plan fund in the best way to provide participant with most benefit.
How it works
As a service provider, we engage with participant to discuss their plan, current issues, goals, services to be provided and fund. Aiming to meet participant needs and achieve plan goals.
What is included in our Plan Managed service?
- We advise regarding best method of utilising the fund clearly for the plan period.
- We provide high quality support workers and allied health.
- We facilitate overall communication between all involved parties in the plan.
- We arrange for regular professional reports to be submitted to NDIS.
